PSMA-Targeted Care Is Diversifying on Both Ends

Diana’s opening noteThis week felt like two different conversations about the same disease. One is about a T-cell engager that reached Fast Track without a single radioactive atom. The other is about the first radioligand therapy now approved for earlier-stage prostate cancer, and what that means once the initial excitement settles into actual referral volume. I think both are worth your attention, for different reasons.

Editor’s Spotlight

Two signals arrived from opposite ends of the PSMA pipeline this week: the FDA granted Fast Track designation to Vir Biotechnology and Astellas’s non-radioactive, PSMA-targeted T-cell engager VIR-5500 for late-line mCRPC, while a Journal of Nuclear Medicine study showed PSMA-PET-guided biopsy nearly doubling cancer detection in men whose MRI missed it. Treatment and diagnosis are both getting sharper at once, and neither development depends on adding another lutetium slot to the schedule.

Story of the Week

Cleveland Clinic published a clinical perspective on Pluvicto’s FDA approval for mHSPC, featuring Dr. Shilpa Gupta, a PSMAddition principal investigator and genitourinary oncologist at Cleveland Clinic Cancer Institute, the trial’s highest-enrolling U.S. site. The FDA approved 177Lu-PSMA-617 (Pluvicto) plus an ARPI for PSMA-positive metastatic hormone-sensitive prostate cancer on July 31, 2026, based on the phase 3 PSMAddition trial published in The Lancet; the interim analysis showed a 28% reduction in risk of radiographic progression or death (HR 0.72, P=.002) against ADT plus ARPI alone. It is the first radioligand therapy approved in this earlier-stage setting, and the October 9 piece is the first institutional read we’ve seen on what that means for referral pathways.

Clinical News

A Journal of Nuclear Medicine study found PSMA-PET/CT-targeted biopsy detected clinically significant prostate cancer in 37% of men with negative or inconclusive MRI, versus 21% with systematic biopsy alone; SNMMI estimates 20–30% of significant cancers are missed under MRI-first protocols.

The FDA granted Fast Track designation to Vir Biotechnology and Astellas’s VIR-5500 for late-line mCRPC; phase 1 data showed an 82% PSA50 response rate and a 45% objective response rate at the highest dose, with grade 3+ events in 12% of patients.

Clarity Pharmaceuticals completed enrollment of its SECuRE trial’s Cohort Expansion main group, each of 16 participants receiving up to six cycles of 8 GBq 67Cu-SAR-bisPSMA; earlier data at that dose showed 81% disease control and 5 complete responses.

Business & Industry Intelligence

GE HealthCare agreed to buy PET radiopharmaceutical manufacturer SOFIE Biosciences for $945 million in cash, gaining 15 U.S. cyclotron sites and U.S. rights to FAPI-74, a phase 3 pan-cancer PET radiotracer; the deal is expected to close in the first half of 2027.

Telix reported its first U.S. commercial doses of Pixclara (floretyrosine F 18), its FDA-approved brain cancer imaging agent, about three weeks after FDA approval — the company’s first U.S. product outside prostate cancer imaging.

Jubilant Radiopharma will invest CAD 94.5 million to modernize its Kirkland, Quebec biomanufacturing site, backed by a CAD 23.8 million Government of Canada grant, creating or maintaining 225 jobs.

Practice insightModel the MRI-negative biopsy pathway now, not after volume shows up.With PSMA-PET-guided biopsy nearly doubling detection in MRI-negative men, forecast added imaging and targeted-biopsy volume, confirm scheduling capacity ahead of EANM and SNMMI season, and align urology referral criteria with the new evidence before it becomes the standard workup.

Clinical Trials to Watch

AcTFirst (Phase 3) — 225Ac-PSMA-617 plus an ARPI vs. investigator’s choice of standard of care in taxane-naive, radioligand-naive PSMA-positive mCRPC; enrollment ongoing.

PSMAddition / Novartis (Phase 3) — 177Lu-PSMA-617 plus ADT and an ARPI in PSMA-positive mHSPC; FDA-approved July 31 as Pluvicto for this indication, with overall-survival follow-up still maturing.

LPS-Boost (Phase 2) — intensified 177Lu-PSMA-617 dosing for mCRPC patients with low PSMA-expressing disease, a population usually excluded from radioligand therapy.

FAPI-74 / GE HealthCare-SOFIE (Phase 3) — pan-cancer PET radiotracer picked up in this week’s SOFIE acquisition; GE already holds rights outside the U.S. and now gains the U.S. program.

Industry Movers & News in Brief

Lantheus issued supplemental disclosures on its $8 billion Curium merger proxy on October 2, after three shareholder suits and 16 demand letters challenged the deal’s valuation disclosures, clearing the way for the October 14 shareholder vote.

ITM and Lumara Bio will present new 177Lu-edotreotide and Ga-DPI-4452 data at EANM’26, including phase 3 COMPETE trial dose-response results.

Telix will host a Capital Markets Day in Melbourne on November 4, its first since announcing the $1.65 billion ITM acquisition.

Upcoming events: EANM’26 Annual Congress October 17–21, Vienna, Austria; ESMO Congress 2026 October 23–27, Madrid, Spain; the SNMMI Theranostics Conference November 5–7, Rockville, MD; and RSNA 2026 November 29–December 3, Chicago, IL.

Diana’s takeThe more interesting story this week isn’t a new radioligand, it’s that PSMA-targeted care is diversifying on both ends: a T-cell engager earned Fast Track status without a single radioactive atom, while plain PSMA-PET got sharper at finding cancer MRI missed. Programs built only around lutetium-based therapy should start tracking non-radioactive PSMA platforms now, not after they reach phase 3 — and should start modeling mHSPC referral volume now that Pluvicto has a label for it.
